How to fill out the Form W-9
To help you complete the W-9 form correctly, we’ve prepared a simple, step-by-step guide below:
In field 1, enter the taxpayer’s name: this is the person or company that provides or provided the service. This can be either an individual or a business entity.

If the business is a single-member LLC, field 1 must include the owner’s name, and field 2 must include the LLC’s legal name.

In field 3, you must select the taxpayer type according to the entity’s structure. The options include Individual, LLC, C Corporation, S Corporation, Partnership, or Trust.

If it is an LLC, you must also indicate how it is taxed. For example:
- LLC taxed as a C Corporation: select “LLC” and enter the letter C.
- LLC taxed as a Partnership: select “LLC” and enter the letter P.

In field 5, enter the address.
In field 6, enter the city, state, and ZIP code.

Next, go to Part I, where you will enter the taxpayer identification number:
- Individual: SSN
or - Business entity: EIN

Finally, sign and date the form, and it will be complete.
